  3. A data-gathering process to determine where the organization is in the transition process.

  4. Sharing the findings from the data-gathering and deciding on an appropriate course of action.

  5. Implementing the transition action plan through training, coaching and facilitating individuals and/or groups.

  6. Monitoring of the transition, evaluation and follow-up.
